The office is located in the Medical Office Building 3 at Mount Carmel West Hospital. Take the Rich St./Town St. exit off SR 315. Continue on Town St. to Green St., turn right. Green St. turns into Mount Carmel Mall as it curves to the left. Our building is on your right.

Take the East Broad St. exit off I-270. Turn north on Taylor Station Road. We are located on the right hand side of the street in the 3 story medical office building.
